Rxpert ESN™
Texas Pharmacy Association’s Enhanced Services Network - Rxpert ESN™, is a health care quality improvement initiative focused on the development and implementation of a high quality community pharmacist network that is integrating their enhanced patient care services with other providers, health systems, and accountable care organizations (ACOs) within Texas. The goal of this high performance community pharmacy network is to optimize patient health outcomes through appropriate medication management and utilization of health care resources.
The network is focused on high quality performance in all aspects of the practice including implementation of enhanced services, provision of high quality patient care, collaboration with other health care providers and key stakeholders, and working closely with payers to improve overall health care resources utilization and costs. This network is prepared to work closely with key stakeholders (i.e. payers, ACOs, health systems, and other health care providers) to address rising health care costs and improve patient health outcomes through collaboration with other providers. We are seeking pharmacies that have demonstrated interest in the delivery of high quality, patient-centered care that exceeds traditional dispensing services to voluntarily participate in this network.
How does the program work?
- Healthcare providers/hospital(s) will develop appropriate protocols/processes to identify high risk patients that would benefit from a referral to a local community pharmacist for adherence focused services.
- Network pharmacists will:
- Initiate contact with patients within 72 hours of referral from hospital/clinic with the preferred method of contact as a face-to-face interview. Telepharmacy may be used when deemed necessary.
- Follow-up with the patient weekly for 2 weeks following initial contact/visit, then four weeks from initial, then eight weeks from initial visit for a total of 4 follow-up interactions.
- Track 60 day hospital readmission associated with uncontrolled chronic hypertension among enrolled patients
- Monitor medication adherence of enrolled patients. Evaluated through medication fill rate, patient interviews/questionnaires and evaluation of remaining pill count (when possible).
- Track patient attrition rates at follow-up intervals during the program.
