Utilizing innovative and secure technology, you, your pharmacist and physician, will be able to communicate effectively and easily.

What will happen once I enroll?

1. Your physician has referred you to a local, specially-trained pharmacist who will help you lower your blood pressure.
2. The pharmacist will help manage your medication while helping you learn to self-manage your blood pressure therapy.
   • You will have one initial appointment with your pharmacist followed by four follow-up appointments.
     

How much will it cost?

There is NO out-of-pocket cost to you when you meet with the specially trained, local pharmacist on a regular basis over the next two to three months.
