The TMF Quality Innovation Network Quality Improvement Organization (QIN-QIO) works collaboratively with health care providers and organizations to accomplish better care, better health for people and communities and more affordable care through quality improvement. The TMF QIN-QIO has been under contract with the Centers for Medicare & Medicaid Services (CMS) to engage health care providers, patients, caregivers and community partners to implement lasting quality improvement best practices. These efforts have led to a more educated and confident health care staff and better health outcomes for all Medicare beneficiaries. The Immunization Project's main goal is to improve immunization rates for older adult patients in Texas. The project focuses in the reduction of preventable illness like flu, pneumonia and herpes zoster, among others. It works hand in hand helping pharmacies raise vaccination rates, as well as encouraging education to highlight prevention. It provides free tools and assistance to improve immunization interventions for older adults and help reduce mortality rates.
Immunization ToolkitHaving the right tools and information from pharmacies is a key element to help patients. TMF QIN-QIO has provided easy-to-read material on immunizations to improve the number of vaccinated adults. The project offers free guidelines, educational tools for patients, and support services for pharmacies and pharmacy team to increase awareness and participation, among others.
