Medicare Transaction Facilitator (MTF)

CMS logoPharmacies and dispensing entities should prepare for the implementation of the Medicare Transaction Facilitator (MTF), a system designed to manage data exchange needed for processing claims and reimbursements under the Medicare Drug Price Negotiation Program. Notifications with enrollment instructions will be sent to pharmacies on a rolling basis beginning June 9, 2025. Below are resources to stay informed about enrollment procedures, system requirements, and training opportunities to ensure seamless integration into this program. The timing of reimbursements on cash flow is a key concern for participating pharmacies, and manufacturers are expected to submit an effectuation plan for how they will address cash flow concerns for pharmacies by September.

Medicare Drug Price Negotiation Program (MDPNP)

The Medicare Drug Price Negotiation Program, established under the Inflation Reduction Act (IRA), enables Medicare to negotiate prices for select high-cost prescription drugs, aiming to reduce costs for beneficiaries and the federal government. Starting January 1, 2026, the first negotiated prices for ten Part D drugs will take effect, with plans to expand the list in subsequent years.

Medicare Prescription Payment Plan (M3P)

The Texas Pharmacy Association is committed to helping our members understand the new Medicare Prescription Payment Plan (M3P) program that may affect their patients. This optional program was established by the federal Inflation Reduction Act (IRA), which included several provisions that impact Medicare Part D beginning January 1, 2025.

Under the M3P program, Medicare patients can elect to participate to spread their out-of-pocket costs more evenly throughout the plan year. Patients most likely to benefit from the program are those that have high out-of-pocket costs early in the plan year. Once a patient has elected to join the program, they will pay no out-of-pocket costs at point of sale and will be billed in monthly payments by their health plan.
