Pharmacists play a critical role in patient care by identifying opportunities to safely reduce or discontinue medications. This course explores key patient characteristics and high-risk medication classes, introduces a step-by-step deprescribing process, and provides practical communication strategies to effectively engage both patients and providers in deprescribing interventions.
